Participant must have completed any of the below Driver’s Safety Course successfully under licensed instructor of National Safety Council Driver Safety Centre: Alive at 25, Attitudinal Dynamics of Driving (ADD), Chronic Offender, DDC Online, DDC 4, DDC 8/6, Rage SCARR, Professional Truck Driver (PTD) and State Courts Against Road.
Participants must meet the eligibility criteria to be a member of the National Safety Council Driver Safety Centre. Check the official website www.nsc.org or refer to your training manual.

You will be directed to the survey page. Choose the name of the course you have completed from the given option. If your course name is not mentioned, select others and specify the name of the course.
Click on Continue.

Enter your Instructor’s Number.
If you do not know Instructor’s Number, then click on the checkbox beside Don’t know Instructor ID Number.
Entre Instructor’s name.
Specify the starting date and end date of the course.
From here, you need to start answering the questions for the survey. The questions are included related to the details of the qualities and knowledge required to be qualified as a safer driver.
At the end of the survey, you will be redirected to the page, as your consent for entering the survey.
