On the main page in the middle type,

• The username

• The password

• Then, click on, ‘Login’ in orange.

How to Recover Your DaVita Login Credentials

If you are looking for help logging in click on, ‘Need help’. On the next directed page click on, ‘Forgot username’.

At the change type,

• The last name

• The date of birth

• The last of SSN

• Then, click on, ‘Next’ in blue.

For password input,

• The username

• Then, click on, ‘Next’ in blue.

To change passwords, type,

• Username

• Current Password

• New Password

• Confirm Password

• Then, click on, ‘Change password’ in yellow.

You can also update your security questions

To update the security authentication type,

• Employee ID

• Last Name

• Date of Birth

• Last 4 of your SSN

• Password

• Then, click on, ‘Next’ in blue.

You have to check the prompts after this to get all these details back.
